Kristin Maack, RN, MHA, NBC-HWC, is a passionate healthcare professional dedicated to transforming patient care through the power of Lifestyle Medicine. With a background that bridges clinical nursing and operational leadership, she brings a uniquely well-rounded perspective to the field.
Currently serving as Director of Practice Transformation at On Belay Health Solutions, Kristin works at the forefront of reshaping how healthcare practices integrate Lifestyle Medicine principles and value-based care strategies. Her previous experience as a registered nurse in a Lifestyle Medicine clinic, delivering hands-on care management and health coaching, and as Director of Operations for a Lifestyle Medicine ACO gives her a deep, ground-level understanding of both patient care and systems-level change.
In her current role, Kristin partners with practices to redesign care delivery models, align clinical workflows with value-based performance goals, and build the infrastructure needed to sustain Lifestyle Medicine at scale. Her ability to translate clinical insight into operational strategy makes her a trusted guide for practices navigating the shift toward whole-person, value-driven care.
