Outdoor Behavioral Healthcare as a High Impact Intervention for Adolescents

Presentation Summary: This session uses a case study and evidence-based lecture format to explore Outdoor Behavioral Healthcare (OBH) as an innovative, nature-based intervention aligned with the six pillars of Lifestyle Medicine, with emphasis on stress management, physical activity, and social connection.

Adolescent mental health is in crisis, with suicide ranking as the second leading cause of death among individuals ages 15–25, and the vast majority affected by underlying mental health disorders. In this context, OBH emerges as a promising, evidence-based therapeutic approach that integrates structured outdoor experiences into clinical care.

We will review landmark research from the University of New Hampshire demonstrating that adolescents (ages 12–17) participating in OBH programs experienced nearly three times greater improvement at one year compared with those receiving community-based “treatment as usual,” with benefits sustained up to 14 months post-treatment across demographic groups.

The session will also examine the physiological and psychological mechanisms underlying nature immersion, including reduced sympathetic nervous system activation after brief forest exposure, improved immune function through increased natural killer cell activity linked to phytoncides, and measurable improvements in adolescent anxiety, attention, and emotional regulation.

Despite strong evidence of effectiveness, OBH remains underutilized due to limited insurance coverage and reimbursement barriers. The session concludes with a “business of medicine” discussion on how Lifestyle Medicine clinicians can advocate for OBH as a reimbursable, equity-enhancing intervention capable of reducing the long-term burden of adolescent mental health disorders.
