Building a Rural LM Service Line: Proctoring, Privileging, and Practice Design

Presentation Summary: This panel session explores the development and launch of a lifestyle medicine (LM) service line within a rural, nonprofit healthcare system serving Central Appalachia—an area with some of the nation’s highest rates of chronic, lifestyle-related disease. Faced with significant health disparities and a predominantly Medicare/Medicaid population, system leadership prioritized a scalable, prevention-focused approach to improve outcomes and workforce wellness.

Through the perspectives of a lifestyle medicine provider, a Regional Chief Medical Officer, and an external board-certified LM physician proctor, attendees will gain a comprehensive understanding of the strategic, clinical, and operational decisions involved in building a dedicated LM service line. Panelists will discuss why a service line model was chosen over embedding LM within primary care, and how leadership navigated privileging requirements, provider alignment, and long-term scalability.

The session will highlight key components of implementation, including clinic design, workflow development, and the role of proctoring in supporting early patient care and provider training. Real-world case examples from the clinic’s initial launch will illustrate how structured coaching and collaborative learning shaped visit design, patient outcomes, and program success.

Panelists will also share innovative approaches to provider recruitment, community engagement, and program growth—particularly in regions with limited access to LM-certified clinicians.
